06 — TERMS, WAIVER AND CONSENT

Please read the following carefully before signing.

1. Nature of Activity. I understand that Brazilian Jiu Jitsu (BJJ) is a contact martial art that involves grappling, takedowns, ground fighting, and submission techniques. I acknowledge that participation in BJJ training carries inherent risks of physical injury including but not limited to bruises, sprains, strains, joint injuries, and in rare cases more serious injury.

2. Assumption of Risk. I, as the parent or legal guardian of the child named in this form, voluntarily consent to my child participating in BJJ training sessions with BEjj Lifestyle operated by AleBEjj. I acknowledge and accept the risks associated with this activity on behalf of my child.

3. Waiver of Liability. To the fullest extent permitted by Australian law, I release and discharge BEjj Lifestyle, AleBEjj, and any associated coaches or assistants from any and all claims, demands, actions, or causes of action arising out of or related to any loss, damage, or injury that may be sustained by my child during training sessions, except where such loss, damage, or injury is caused by gross negligence or wilful misconduct.

4. Medical Treatment Authority. In the event of an emergency where I cannot be contacted, I authorise AleBEjj to seek and consent to any necessary emergency medical treatment for my child. I agree to be responsible for any costs associated with such treatment.

5. Supervision and Conduct. I understand that my child is expected to follow the instructions of the coach at all times and to behave in a respectful and safe manner. BEjj Lifestyle reserves the right to remove any student from training without refund if their behaviour poses a risk to themselves or others.

6. Photography and Media. From time to time BEjj Lifestyle may photograph or record training sessions for use on social media, the website, or promotional materials. I may opt out of this by leaving the photography consent box unticked below.

7. Privacy. My child's personal information will be collected, stored, and used in accordance with the BEjj Lifestyle Privacy Policy available at www.bejj.au/privacy-policy. I confirm that I am the parent or legal guardian of the child named in this form and that I am authorised to provide this information on their behalf.

8. Acknowledgement. I confirm that the information provided in this form is accurate and complete to the best of my knowledge. I agree to notify BEjj Lifestyle promptly of any changes to my child's medical status or contact details.

Training Consent (Required). I consent to my child participating in BJJ training sessions with BEjj Lifestyle and acknowledge the inherent risks as described above.Medical Treatment Authority (Required).
